Customer Application Form
To protect stained glass professionals who earn their living by
selling into the retail market, we have established a firm policy
for qualifying new wholesale customers. The requirement is to be
a for-profit business related to Glass, Crafts, or Fabrication of
products using Glass. Meeting this requirement must be documented
by submitting a completely filled out customer application and by
providing copies of three items, at least ONE from each group listed
below.
- Group A: A Tax/Resale Certificate or Business License
- Group B: Professionally Printed Letterhead or Professionally
Printed Business Card
- Group C: Business Phone with Yellow Page Listing or
Checking Account in the Name of the Business
The acceptability of provided documentation will be determined solely
by the Art Glas House. Additional documentation may be required at
our descretion.
The minimum first order for new customers is $700. This minimum
may be waived with presentation of a photocopy of a recent invoice,
totaling $700 or more from a stained glass wholesale distributor
with similar customer requirements.

start the application process. To complete the process, please print,
sign, and mail the application, along with copies of supporting
